10/4/2023 EnchantaVaughn Approved Behavior: QMHP-A arrived for the session and greeted Ms. Cosby. QMHP-A assessed Ms. Cosbyâs current stress/anxiety levels. QMHP-A inquired about her day. She verbalized to QMHP-A her day was going well. Ms. Cosby did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHP-A discussed with Ms. Cosby the effects of isolative behaviors and how being isolated could cause her to slip into a deeper depression. QMHP-A accompanied Ms. Cosby through an enjoyable activity where she was able to interact with others and enjoy herself. QMHP-A engaged Ms. Cosby in a discussion surrounding situations that causes her to become depressed or said and how she deals with the situation. QMHP-A conducted a skills building activity. QMHP-A explained to Ms. Cosby that the purpose of this activity was to demonstrate with to Ms. Cosby coping mechanism through roles play. QMHP-A explained to Ms. Cosby to apply coping skills when she is facing anxiety or depression. QMHP-A guided Ms. Cosby through breathing techniques, exercises, walking, writing exercises and verbalization techniques. QMHP-A discussed with Ms. Cosby the importance of being able to express her feelings and displaying positive behaviors. Response: Ms. Cosby appeared to be receptive to QMHP-Asâ interventions and services that were provided to her during the session. Ms. Cosby expressed to QMHP-A that she knows that isolating herself at times is not something that she needs to do, but it is the best thing to do when it comes to her depressive state. Ms. Cosby said she will listen to music when depressed or walk around. Ms. Cosby said she just feel like walking when questioned about her mental illness. During the skills building activity, Ms. Cosby was able to provide QMHP-A with ways that she deals with depression and anxiety; however, she admitted to QMHP-A that she doesnât apply those skills at all. Plan: QMHP-A will teach the client relaxation skills (e.g., progressive muscle, guided imagery, slow diaphragmatic breathing) and how to discriminate better between relaxation and tension; teach the client how to apply these skills to her daily life. (1-2x weekly).
